The Foundation for Health Care Continuums serves the specialized housing, hospitality and healthcare needs of an ever-evolving market by pioneering new products and services that anticipate the needs, wants and desires of our elders.

The aging of America's population has only just begun, and the future for nonprofit organizations offers both challenges and opportunities. As our society begins to question the way we provide for the elderly, we believe there is an opportunity to transform the way services are provided to the elderly that will last for generations to come. Nonprofit organizations are leading the way in this transformation. We see opportunities to develop new services to a younger, healthier and more active population that focuses on wellness. We strive to develop new models to deliver skilled nursing care for the very sick and frail population.

As the baby boom generation begins to reach retirement age, its members want continued vitality with meaningful purpose. They do not envision a lifestyle in which they are cut off from the culture and society. They care about the environment, education and technology. They expect to remain a vibrant part of our society regardless of their age.

Our vision is driven by a desire to improve and change the existing senior living service delivery models. We believe the current system is broken and only a new system will offer the level of autonomy and dignity our elders deserve. We believe our minds, bodies and spirits are best served when we have the ability to make choices freely. We want to become a catalyst for change by embracing new ways to meet the evolving needs of an aging population as we cope with illness, loss and death.

Wellness and the concepts behind it have moved to the forefront of health promotion and disease prevention. Innovative practices are being identified and applied to senior residences and community programs. The Foundation is evaluating pioneering programs that address the needs, interests, and well-being of older adults.
